Most physicians understand the importance of documenting services effectively; still they end up missing out important things that should be documented. Clinical Documentation Improvement has become crucial for reimbursements, especially since physicians are increasingly undergoing pre- and post-payment audits and increased third-párty scrutiny.

Even though you have been getting paid for your services, it does not mean that the payment will continue, unless your documentation supports the services you provided. You need to understand that your payment could be affected by the lack of specificity.
