Applicants interested in sitting for the CAP exam must meet the following eligibility criteria:

Doctor of Medicine (MD) and Doctor of Osteopathic Medicine (DO)

  • Potential applicants must provide a copy of their current, unrestricted and valid medical license.
  • Potential applicants must complete an approved legal history form (honor/attestation statement form) setting forth any findings made against them in professional malpractice, civil actions that may reflect upon competence or moral fitness to practice, or medical board or hospital or health facility disciplinary consent agreements entered into or orders against them in any state in which they have practiced a health care profession.

Naturopathic Doctor (ND)

  • Potential applicants must provide a copy of their current, unrestricted and valid naturopathic license.
  • Potential applicants must submit with their application a copy of the state law governing the scope of practice with respect to ordering and delivering IV therapies and the IV guideline regulations for the state they practice naturopathic medicine in.
  • Potential applicants must complete an approved legal history form (honor/attestation statement form) setting forth any findings made against them in professional malpractice, civil actions that may reflect upon competence or moral fitness to practice, or medical board or hospital or health facility disciplinary consent agreements entered into or orders against them in any state in which they have practiced a health care profession.

Nurse Practitioner (NP)

  • Potential applicants must provide a copy of their current, unrestricted and valid medical license.
  • Potential applicants must submit with their application a copy of the state law governing the scope of practice with respect to ordering and delivering IV therapies and the IV guideline regulations for the state in which they practice nursing.
  • Potential applicants must complete an approved legal history form (honor/attestation statement form) setting forth any findings made against them in professional malpractice, civil actions that may reflect upon competence or moral fitness to practice, or medical board or hospital or health facility disciplinary consent agreements entered into or orders against them in any state in which they have practiced a health care profession.
